New patient information
We look forward to seeing you at Novant Health Coastal Plastic Surgery. To schedule a consultation, call 910-754-8808.
Please arrive at least 15 minutes before your appointment. Bring with you:
- A photo ID.
- Your insurance card.
- Your method of copayment or payment in full if your visit is not covered by insurance. (Click here for insurance and billing information.)
- Any prescription or over-the-counter medications you are currently taking.
Our office will work to get any relevant medical records from other providers prior to your visit. However, if you have recent test results or other records in your possession, please bring those with you.
